CHIF Grant Application Guidelines and Deadlines PDF Print E-mail
 

Community Health Improvement Fund (CHIF) grants are intended to provide seed money, start-up funding, or venture capital to organizations seeking to address emerging or critically important health issues, start new programs, demonstrate the effectiveness of a program on a pilot basis, or implement an established best practice.  CHIF grants are one-time awards of up to $40,000 for a term of up to 24 months.  They are not renewable. 

Organizations applying for a Community Health Improvement Fund Grant (CHIF Grant) must address either of the following priorities:

  • Access to necessary health services with particular emphasis on eliminating the barriers often encountered by persons in need
  • Wellness, with particular attention to substance abuse, including tobacco; HIV/AIDS and STDs; adolescent pregnancy prevention; and mental health.
In addition to these priorities, the Foundation has a strong and special interest in funding organizational development and technical assistance projects that help communities build the capacity to help themselves improve the health of those in their communities.

Eligibility
In addition to meeting the criteria above, your organization must serve primarily the residents in the geographic area served by Moses Cone Health System and be one of the following:
  • Not-for-profit organization
  • Government agency
  • School
  • Academic and research institution
  • Collaborative and consortia

If you meet the eligibility requirements above and you are seeking funding up to $40,000 per year, you may apply for a CHIF Grant. 

Please note that these are one-time grants and are not renewable, but the term of a CHIF grant may extend up to 24 months. However, regardless of the term, the maximum amount available is $40,000 per program.

Step One: Submit a Letter of Interest
The Foundation requires those interested in the CHIF Grant to submit a Letter of Interest no longer than two pages to:

Generally, the letter should include (please see current RFP for specific grant requirements each cycle):
  1. a summary of the problem
  2. a brief description of the project or program proposed to address the problem
  3. a brief description of the ways in which the success of the project or program will be measured or evaluated
  4. the total estimated cost of the project or program, and the approximate amount of support to be requested from the Foundation
  5. a brief description of the organization, including key staff members and any characteristics or qualities that make the organization particularly well-suited to undertake the proposed project or program
  6. names of collaborating organizations, including other organizations from which financial support is expected or requested for the proposed project or program
  7. complete application contact information including address, telephone, fax, email and web site address if applicable.
Step Two: Invitation to Submit an Application
The Foundation will review Letters of Interest, and organizations and programs that meet the eligibility requirements and funding priorities of the Foundation will be contacted by the Foundation and invited to submit a full application for support. (Instructions can be give at that time about how to download the application online.) Other inquiries may be declined, deferred to a future funding cycle, or referred to other potential funding sources.

Applications in response to invitations by the Foundation are due in our office no later than the established cycle deadline. Late submissions will not be accepted. Incomplete applications will be returned, although the submitting organization will be invited to resubmit the application at a future date.

Step Three: Application Evaluation and Approval
Applications are reviewed by the Foundation staff and Board of Directors. Foundation representatives typically visit applicant organizations during the review process in order to learn about the project first hand. Decisions of approval are communicated to the applicant on or before the award date specified in the grant cycle. Initial grant payments for approved applications are contingent upon satisfactory completion by the applicant of the Grant Acknowledgement Guidelines.
