For the past 10 years, the Moses Cone-Wesley Long Community Foundation has strategically worked in the community guided by principles and values. These values reveal how the Foundation views itself—as convener, catalyst and leader. These principles focus on how the Foundation interacts with others, valuing individual and organizational expertise for input, collaboration in helping the Foundation achieve its mission. And, most importantly, about how the Foundation and its partners complete their work:
- Avoiding duplication of services that wastes precious resources;
- Closing the gaps in access to care for all, creating a seamless continuum of services;
- Seeking out ways to assist agencies to leverage Foundation funding to obtain finances and resources from the broader community, from other foundations, from state and federal grants;
- Strategically focusing effort on initiatives that promote systemic and community-wide change, while still striving to recognize and address the immediate needs of individuals; and
- Maintaining accountability, accountability of the grantees to assure project success and to document that success, and maintaining the accountability of the Foundation itself, making sure that the Foundation achieves its mission, fulfills its vision, is true to its values and upholds its fiscal and programmatic responsibility.

A Decade of Data Confirms Foundation Success
Achieving better overall health in a community requires broad, dependable, and interconnected participation by individuals and organizations. A solid decade of data confirms the success of the Foundation and its partners—teenage pregnancy rates are lower, access to medical care is greater, more people in our area are benefiting form regular physical activity while fewer are smoking. But while it is important that we measure our results with cumulative numbers, the most inspiring evidence of a healthier community is always the individual lives made better. The real stories of real people are what give the Foundation confidence in our mission, and experience has taught us that success is the result of a community working together. The Foundation pledges to continue its efforts to promote education, expand awareness and to help effectively coordinate health resources to improve the health, safety, and overall wellness of everyone in the community.
