Partnership at its Best--
The Community Care Network
Many residents in our community are unable to seek care because they do not have access to a network of routine primary care givers. Since 2000, Moses Cone-Wesley Long Community Health Foundation has partnered with safety-net providers—organizations caring for local residents who do not have insurance or are uninsured—to address issues such as eligibility standards, sharing of patient records and improving access to specialty health care services. The ultimate goal is to provide access to care and a medical home for all residents of our community.
