Community Health Improvement Fund (CHIF) Grants PDF Print E-mail

Organizations and projects seeking funding up to $40,000 and meet the criteria specified in the funding priorities and eligibility guidelines may be qualified to apply for a CHIF Grant. This program provides grants to organizations that, on their own initiative, are consistent with the mission, purpose and funding priorities of the Foundation. In addition, this funding is restricted for support of new programs that address emerging or priority health needs, programs that demonstrate new and innovative approaches to addressing such needs, or to provide start-up funding for demonstrated best practices. CHIF Grants may be awarded up to three years.

Through the CHIF Grant program, Moses Cone-Wesley Long Community Health Foundation seeks to provide funding to organizations and programs that address either of the following priorities.

Funding Priorities

Access to necessary health services with particular emphasis on eliminating the barriers often encountered by persons in need, or

Wellness, with particular attention to:

  • physical activity
  • nutrition/obesity
  • substance abuse, including tobacco use prevention/cessation
  • responsible sexual behavior, especially the prevention of HIV/AIDS, STDs, and adolescent pregnancy
  • infant mortality
  • mental health

In addition to these priorities, the Foundation has a strong and special interest in funding organizational development and technical assistance projects that help communities build the capacity to help themselves, thus being able to improve the health of those in their communities.

Eligibility
In addition to meeting the criteria above, your organization must serve primarily the residents in the geographic area served by Moses Cone Health System and be one of the following:

  • Not-for-profit organization
  • Government agency
  • School
  • Academic and research institution
